Tirzepatide's Pancreatic and Gallbladder Safety Profile in Obesity Management Systematically Mapped Across Diverse Evidence

Pancreatic and Gallbladder Adverse Events Associated with Tirzepatide in Obesity Management: A Scoping Review.

Background

Obesity is a pervasive, chronic condition driving significant comorbidities and healthcare costs. While tirzepatide, a novel dual GIP/GLP-1 receptor agonist, offers substantial weight reduction, its comprehensive safety profile, particularly concerning pancreatic and gallbladder adverse events (AEs), remains under scrutiny. Current understanding lacks a systematic synthesis across varied evidence sources, leaving clinicians and patients without a clear, consolidated view of these specific risks. This gap necessitates a thorough mapping of existing data to inform safer therapeutic strategies.

Study Design

This scoping review systematically mapped evidence on tirzepatide's safety in obesity, focusing on pancreatic and gallbladder AEs. Following Arksey and O'Malley's framework, expanded by Joanna Briggs Institute guidance, researchers included 11 RCTs, 5 observational studies, 4 case reports, 1 EudraVigilance pharmacovigilance analysis, and 575 FAERS reports. Data was identified from PubMed, EMBASE, Cochrane Library, FDA Adverse Event Reporting System (FAERS), ClinicalTrials.gov, gray literature, and targeted journal searches between December 2019 and September 2025. Pancreatic and gallbladder AEs were extracted and coded using MedDRA version 28.1.
