Background

Periapical infections, often leading to periapical periodontitis (inflammation and infection around the tooth root), are a significant challenge in dentistry. These infections are typically polymicrobial, involving a complex array of bacteria that can be difficult to eradicate with conventional treatments, leading to persistent symptoms and treatment failures. Current antimicrobial strategies, while effective, face issues like bacterial resistance and limited penetration into the complex root canal system. Results

The study revealed that LL-37 peptide exhibited significant antimicrobial activity against all tested periapical pathogens in a concentration-dependent manner. The MIC50 values for most strains ranged between 2 µM and 8 µM, indicating potent inhibitory effects. Specifically, LL-37 at 10 µM achieved a 90% reduction in viable Porphyromonas gingivalis cells within 24 hours (p<0.001) compared to untreated controls. Furthermore, at 20 µM, the peptide demonstrated bactericidal activity, reducing Enterococcus faecalis counts by 99% (p<0.001). > The most impactful finding was LL-37's ability to significantly inhibit and disrupt mature bacterial biofilms, reducing biofilm biomass by 65% at 10 µM (p<0.01) across multiple species, a 2.5-fold greater reduction than standard chlorhexidine at comparable concentrations.
