CJC-1295 outperforms hGRF(1-29) with 4-fold GH increase and 72-hour plasma presence

Human growth hormone-releasing factor (hGRF)1-29-albumin bioconjugates activate the GRF receptor on the anterior pituitary in rats: identification of CJC-1295 as a long-lasting GRF analog.

Background

Growth hormone deficiency and suboptimal GH axis stimulation represent a clinical gap where short-acting peptides like native hGRF(1-29) are rapidly degraded before achieving sustained physiological effect. The key culprit is dipeptidylpeptidase-IV (DPP-IV), a serum protease that cleaves GRF analogs at the N-terminus, sharply curtailing plasma half-life. The GRF receptor on the anterior pituitary is the primary target for stimulating endogenous GH secretion, making it a desirable therapeutic node. Extending peptide residence time by tethering to endogenous serum albumin via its free thiol at Cys34 is a bioconjugation strategy that bypasses renal filtration and protease exposure, potentially converting a short-burst secretagogue into a long-acting therapeutic.

Study Design

Three maleimido derivatives of hGRF(1-29) were synthesized and conjugated to human serum albumin ex vivo, then evaluated in a GH secretion assay using cultured rat anterior pituitary cells. All three conjugates were tested for in vitro stability against DPP-IV. The compounds were administered subcutaneously to normal male Sprague-Dawley rats, with plasma GH measured over a 2-hour acute window. CJC-1295, selected as the lead compound, underwent extended pharmacokinetic evaluation with plasma sampling beyond 72 h. A Western blot analysis of rat plasma was performed to confirm albumin-bound CJC-1295 immunoreactivity over time.

Results

All three human serum albumin conjugates demonstrated enhanced in vitro stability against DPP-IV degradation compared with unconjugated hGRF(1-29), and all were bioactive in the rat anterior pituitary cell GH secretion assay. Acute subcutaneous administration of each maleimido derivative to Sprague-Dawley rats triggered measurable GH secretion in plasma.

CJC-1295 produced a 4-fold increase in GH area under the curve over a 2-hour period compared with hGRF(1-29), making it the best-performing of the three analogs.

CJC-1295 is a tetrasubstituted hGRF(1-29) bearing an added Nε-3-maleimidopropionamide derivative of lysine at the C-terminus. Pharmacokinetic profiling revealed CJC-1295 was still detectable in rat plasma beyond 72 hours post-injection. Western blot analysis confirmed a CJC-1295-immunoreactive species co-migrating with the serum albumin band, appearing as early as 15 minutes post-injection and persisting beyond 24 hours, directly demonstrating in vivo albumin bioconjugation.

Key Findings

  • CJC-1295 produced a 4-fold increase in GH AUC over 2 h vs. hGRF(1-29)
  • CJC-1295 remained detectable in rat plasma beyond 72 h post-subcutaneous injection
  • Albumin-bound CJC-1295 immunoreactivity appeared within 15 min and persisted beyond 24 h
  • All three albumin conjugates showed enhanced DPP-IV stability and GH bioactivity in vitro
  • CJC-1295 is a tetrasubstituted hGRF(1-29) with a C-terminal maleimidopropionamide-lysine addition

Why It Matters

CJC-1295's ability to bioconjugate to endogenous albumin in vivo after a single subcutaneous dose fundamentally changes the pharmacokinetic calculus for GRF-based therapies — a single injection may sustain GH axis stimulation across days rather than minutes. For clinicians and researchers exploring GH secretagogue protocols, this suggests dosing intervals far longer than those required for native GHRH or sermorelin, potentially improving compliance and reducing injection burden. The confirmation of active GRF receptor engagement alongside extended half-life means the compound is not merely stable but functionally durable. Translation from rat models to human protocols still requires bridging PK/PD data, but the albumin-conjugation mechanism is conserved across mammalian species, strengthening the translational rationale.
