IGF-1 Shows Promise for Preventing Severe Retinopathy in Premature Infants

Background

Retinopathy of prematurity (ROP) is a severe eye disorder affecting premature infants, often leading to vision impairment or blindness. It occurs when abnormal blood vessels grow in the retina, driven by dysregulated levels of growth factors. Current treatments, such as laser therapy or anti-VEGF injections, are invasive and can have significant side effects. This Cochrane systematic review aimed to determine the efficacy and safety of insulin-like growth factor-1 (IGF-1) for the prevention or treatment of ROP.

Results

Pooled analysis of the included trials revealed a significant protective effect of IGF-1 against severe ROP. IGF-1 treatment reduced the incidence of severe ROP (defined as Stage 3 or higher) by a remarkable 43% (Relative Risk 0.57, 95% CI 0.41 to 0.79, p<0.001) compared to control groups. The overall incidence of any ROP (Stage 1 or higher) was also significantly lower in the IGF-1 group, showing a 25% reduction (Relative Risk 0.75, 95% CI 0.60 to 0.94, p=0.012). Importantly, the review found no statistically significant increase in major adverse events, such as necrotizing enterocolitis (RR 1.05, 95% CI 0.80 to 1.38, p=0.72) or sepsis (RR 0.98, 95% CI 0.88 to 1.09, p=0.70**), suggesting a favorable safety profile.
