Rare Combined Hormone Deficiencies Mimic Chronic Fatigue Syndrome in Middle-Aged Man

Many individuals suffer from debilitating chronic fatigue syndrome (CFS), a complex condition often difficult to diagnose due to its varied and non-specific symptoms. Similarly, fibromyalgia presents with widespread pain and fatigue, further complicating diagnosis. This case report highlights the importance of considering endocrine deficiencies as a differential diagnosis for CFS and fibromyalgia mimics, which can lead to effective treatment.
Results
After starting oral hydrocortisone replacement, the patient experienced significant amelioration of his general fatigue. Following the initiation of GH replacement therapy, his muscle pain and depression also showed marked improvement. This case demonstrates that specific hormone deficiencies can present with symptoms indistinguishable from CFS and fibromyalgia. The combined treatment of hydrocortisone and GH effectively resolved symptoms previously attributed to CFS and fibromyalgia, demonstrating a clear link between hormone deficiencies and symptom resolution. This suggests that a thorough endocrine workup can uncover treatable causes for chronic fatigue and pain.
Why It Matters
This case underscores the critical importance of comprehensive endocrine screening in patients presenting with CFS-like symptoms and fibromyalgia. Identifying and treating underlying hormone deficiencies, such as ACTH and GH deficiency, can lead to significant symptom resolution and improved quality of life. This finding could lead to revised diagnostic protocols for CFS and related conditions, potentially benefiting many patients currently misdiagnosed or undertreated. Future research should explore the prevalence of these deficiencies in broader CFS populations, potentially leading to larger clinical trials.
