Thymosin Alpha-1 Included in Multimodal Treatment of Postpartum COVID-19 ARDS Requiring ECMO

Postpartum extracorporeal membrane oxygenation of a woman with COVID-19-related acute respiratory distress syndrome: A case report.

Background

COVID-19-related acute respiratory distress syndrome (ARDS) carries high mortality and presents particular management challenges in peripartum patients, where physiological changes in blood volume and coagulation compound respiratory failure. Standard ventilatory support frequently proves insufficient, necessitating escalation to extracorporeal membrane oxygenation (ECMO). Reports of veno-venous ECMO in postpartum COVID-19 patients are scarce, leaving clinicians with limited guidance on optimizing coagulation management, fluid balance, and immunological support simultaneously. Thymosin α1 has been explored as an immunomodulatory peptide in viral infections given its capacity to enhance T-cell function, but its role alongside ECMO in this vulnerable population is undescribed.

Study Design

Single-patient case report of a 31-year-old woman admitted at 35 weeks gestation with COVID-19-associated ARDS. An emergency bedside cesarean section was performed. Post-delivery, the patient received thymosin α1 (immunotherapy), antivirals (lopinavir/ritonavir and ribavirin), antibiotics (imipenem-cilastatin sodium and vancomycin), and methylprednisolone. Invasive mechanical ventilation was initiated and replaced by veno-venous ECMO 5 days postpartum. ECMO management prioritized blood volume control, coagulation function adjustment, and airway management. No control arm was present given the single-case design.

Results

The patient presented with obvious dyspnea, chills, fever, and rapidly decreasing arterial oxygen saturation, consistent with severe ARDS secondary to COVID-19. Emergency cesarean section was performed to facilitate maternal resuscitation. Mechanical ventilation was escalated to veno-venous ECMO at 5 days postpartum when ventilatory support proved insufficient. The integrated management strategy — encompassing thymosin α1 immunotherapy, antiviral and antibiotic regimens, corticosteroids, and targeted ECMO care — resulted in successful clinical recovery.

The patient was successfully weaned from both ECMO and mechanical ventilation and made a good recovery.

No adverse outcomes attributable to thymosin α1 were reported, and the multimodal regimen was tolerated across the postpartum ECMO course. Specific laboratory values, ECMO duration, and quantitative oxygenation metrics were not reported in the abstract.

Key Findings

  • 31-year-old at 35 weeks gestation developed COVID-19 ARDS requiring emergency cesarean section.
  • Veno-venous ECMO initiated 5 days postpartum after mechanical ventilation proved insufficient.
  • Thymosin α1 included as immunotherapy alongside antivirals, antibiotics, and methylprednisolone.
  • Patient successfully weaned from ECMO and ventilator with good recovery reported.
  • ECMO management focused on blood volume control, coagulation adjustment, and airway management.

Why It Matters

Postpartum patients on ECMO represent one of the highest-complexity critical care scenarios, combining altered coagulation physiology, postoperative hemodynamics, and immune dysregulation driven by COVID-19. The inclusion of thymosin α1 as an immunomodulatory adjunct in this context signals a potential role for peptide-based immune support where viral-induced lymphopenia may blunt recovery. Clinicians managing similar cases should note that blood volume control, coagulation adjustment, and airway management were identified as the core ECMO pillars. For practitioners interested in thymosin α1 stacking within antiviral protocols, this case suggests tolerability alongside lopinavir/ritonavir and corticosteroids, though no efficacy signal can be isolated from a single case report.
