A peer-reviewed study made six test purchases of semaglutide from online sellers that required no prescription. Three vials arrived and were analysed; the three prefilled pens never arrived. Measured purity was 7.7% to 14.37%, semaglutide content exceeded the labelled amount by 28.56% to 38.69%, and endotoxin was detected in all three samples at 2.1645 to 8.9511 EU/mg.

The study is by Ashraf, Mackey, Vida, Kulcsár, Schmidt, Balázs, Domián, Li, Csákó and Fittler, published November 7, 2024. Its sample is three vials, so it shows what those purchases contained, not how common such products are.

Correction, October 1, 2026. This entry previously credited the study to "Ambrus et al." in JMIR Infodemiology with doi 10.2196/52877, described seven samples, and called it the most-cited primary source in the field. The authors, journal, DOI and sample size were wrong, and the superlative was unsupported.
