Reddit analysis reveals distinct patient concerns for bariatric surgery vs. Ozempic

Patient perceptions significantly influence the success of interventions for obesity, including bariatric surgery and pharmacologic therapies like semaglutide. However, many critical concerns often go unaddressed during brief clinic visits, creating a gap in comprehensive patient care. Online communities like Reddit offer an invaluable, unfiltered window into these 'latent concerns,' allowing researchers to explore patient attitudes and experiences outside traditional clinical settings, thereby informing more tailored pre- and post-intervention counseling strategies.

Researchers leveraged natural language processing (NLP) to compare user-submitted content across three Reddit communities: /r/GastricBypass, /r/GastricSleeve, and /r/Ozempic. They extracted the top 500 posts and all associated comments from each subreddit. NLP libraries quantified sentiment and subjectivity, while Latent Dirichlet Allocation (LDA) identified key thematic topics. Statistical significance was determined using Kruskal-Wallis and chi-square (χ2) tests. A machine learning classifier categorized posts as either pre- or post-intervention to analyze evolving concerns.

The analysis encompassed 1500 posts and 45,972 comments, revealing a largely neutral/positive sentiment (0.26 ± 0.35) and moderate subjectivity (0.48 ± 0.33). Comment sentiment was most positive in /r/GS and least in /r/GB (0.30 ± 0.33 vs 0.20 ± 0.31, respectively, P < 0.001), though post sentiment did not significantly vary (P = 0.246). Five main themes emerged: eating behaviors/diet, weight loss progress, medication/medical issues, adverse effects, and support. Distinct adverse effect profiles were observed:

Pain was the predominant adverse effect in surgical subreddits (24.3% within /r/GB and 16.8% in /r/GS), contrasting sharply with gastrointestinal (20.8%) and psychological (7.7%) symptoms reported in /r/Ozempic (P < 0.001). Most content was post-intervention (85.0%), and pre-intervention posts exhibited significantly higher levels of anxiety and depression (4.0% vs 0.9%, P = 0.049).
