Irish Pharmacists Report GLP-1 RA Counseling Gaps, Safety Concerns, and Training Needs for Obesity Management

The global rise in obesity necessitates effective management strategies, with Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) emerging as a key pharmacological intervention. While GLP-1 RAs offer significant weight loss and metabolic benefits, their safe and effective use relies heavily on comprehensive patient education and support. Study Design

Researchers conducted a cross-sectional study in Ireland, distributing an online questionnaire to community pharmacists in July 2025. The survey included both closed-ended questions, analyzed quantitatively using descriptive statistics, and open-ended questions, which underwent conventional content analysis. The study aimed to capture the experiences and perspectives of pharmacists regarding GLP-1 RAs for obesity management, focusing on their counseling practices, perceived barriers, and concerns. A total of 200 completed questionnaires were collected for analysis, providing a snapshot of current pharmacy practice.

Results

Of the 200 pharmacists surveyed, 16.1% (n=31) reported dispensing GLP-1 RAs specifically for obesity, while a substantial 82.9% (n=160) dispensed them for both obesity and diabetes management. Pharmacists frequently provided counseling on practical aspects of GLP-1 RA use, including storage (83.94%, n=162), administration (74.09%, n=143), and side effects (66.32%, n=128). Sharps disposal was discussed by 51.3% (n=99) of respondents. However, counseling on topics like nutrition and contraception was less frequent, with 45.6% (n=88) never providing contraception advice to female patients. Time constraints were identified as the primary barrier to comprehensive counseling. Pharmacists also highlighted significant challenges, including the impact of medication shortages, increased pressure on pharmacy workflows, and concerns regarding safety and prescribing oversight, particularly for online prescriptions. They also expressed a strong need for additional education and training.
