Tirzepatide Lowers Blood Pressure and Improves Heart Function in Hypertensive Rats

Effects of tirzepatide, a dual GIP and GLP-1 receptor agonist, on blood pressure, cardiac function, and sympathetic nervous system in stroke-prone spontaneously hypertensive rats.

Background

Tirzepatide, a novel dual GIP and GLP-1 receptor agonist, is well-known for its efficacy in managing type 2 diabetes and promoting weight loss. However, hypertension and associated cardiovascular complications remain significant challenges, particularly in individuals with metabolic dysfunction. While GLP-1 agonists have shown some cardiovascular benefits, the specific impact of a dual GIP/GLP-1 agonist like tirzepatide on blood pressure, cardiac function, and the underlying sympathetic nervous system activity in a severe hypertensive model has been less explored. This study aimed to evaluate tirzepatide's effects on these critical cardiovascular parameters in stroke-prone spontaneously hypertensive rats (SHRSP).

Results

The study revealed significant cardiovascular improvements in the tirzepatide-treated group compared to controls. Systolic blood pressure (SBP) was markedly reduced, with the tirzepatide group showing a 35 mmHg decrease from an initial 200 mmHg to 165 mmHg by week 8, a statistically significant difference (p<0.001). Echocardiography demonstrated improved cardiac function, including a 12% increase in left ventricular ejection fraction (p<0.05) and a 28% reduction in left ventricular mass index, indicating attenuated cardiac hypertrophy (p<0.01). Furthermore, sympathetic nervous system overactivity was significantly suppressed: > Plasma norepinephrine levels, a key indicator of sympathetic tone, were reduced by 40% in the tirzepatide group compared to controls (p<0.001), suggesting a direct modulatory effect on the autonomic nervous system. These findings collectively point to a robust cardioprotective effect of tirzepatide beyond its metabolic benefits.

Why It Matters

This research highlights that tirzepatide offers substantial cardiovascular benefits, specifically in reducing blood pressure and improving heart function, independent of its known metabolic effects. The observed modulation of the sympathetic nervous system provides a novel mechanistic insight into its cardioprotective actions, suggesting it directly addresses a key driver of hypertension and cardiac remodeling. These findings strongly suggest that tirzepatide could emerge as a powerful therapeutic agent for managing hypertension and preventing severe cardiovascular complications, including stroke, in high-risk human populations. Future research should focus on confirming these mechanisms in human clinical trials and exploring its long-term cardiovascular outcomes.
