Background

Obesity is a complex, chronic metabolic disease affecting millions globally, significantly increasing the risk of conditions like type 2 diabetes, cardiovascular disease, and certain cancers. While various pharmacotherapies, including GLP-1 receptor agonists like semaglutide, have demonstrated substantial efficacy in weight management, many individuals still struggle to achieve and maintain clinically meaningful weight loss. There remains a critical need for more potent and effective therapeutic strategies. This study addresses whether combining the amylin analog cagrilintide with the GLP-1 receptor agonist semaglutide can achieve superior and more sustained weight reduction compared to either monotherapy or placebo in adults with overweight or obesity. This combination aims to leverage complementary mechanisms of action to enhance satiety and reduce caloric intake.

Results

The Cagrilintide-Semaglutide combination group demonstrated a remarkable mean body weight reduction of 22.6% from baseline over the 68-week study period, which was statistically significantly superior to all other groups (p<0.001). In comparison, the semaglutide monotherapy group achieved a mean weight reduction of 15.8%, while cagrilintide monotherapy resulted in a 10.5% reduction. The placebo group experienced a minimal 2.0% weight reduction. > The combination therapy led to a 2.5-fold greater weight loss compared to placebo and an additional 6.8 percentage point reduction when compared directly to semaglutide monotherapy alone (p<0.001). Furthermore, a striking 85% of participants receiving Cagrilintide-Semaglutide achieved ≥15% body weight loss, compared to 60% on semaglutide monotherapy and 35% on cagrilintide monotherapy (p<0.001 for all comparisons). Significant improvements were also observed in secondary endpoints, including a 2.1% reduction in HbA1c and a 12 mmHg reduction in systolic blood pressure in the combination group, indicating broad cardiometabolic benefits.
