GHRH agonist MR-409 reduces myocardial infarction size and inflammation in rats after heart attack

New therapeutic approach to heart failure due to myocardial infarction based on targeting growth hormone-releasing hormone receptor.

Background

Following a myocardial infarction (MI), or heart attack, the heart muscle undergoes significant damage, leading to inflammation, cell death (apoptosis), and scarring (fibrosis). This remodeling process often results in heart failure. Current therapeutic strategies aim to manage symptoms and slow progression but often fail to address the underlying cardiac repair mechanisms. This research builds on previous findings that growth hormone-releasing hormone (GHRH) agonists are cardioprotective. The study investigates a new generation of potent GHRH agonists, targeting the GHRH receptor (GHRHR), to see if they can more effectively promote cardiac repair by mitigating the inflammatory and fibrotic responses that follow an MI.

Study Design

The study used a two-pronged approach. In vitro, H9c2 rat heart cells were cultured in serum-free medium to mimic nutritional deprivation and treated with GHRH agonists to assess cell survival and calcium influx. In vivo, adult male rats underwent surgically induced cardiac infarction and were then treated for four weeks with either placebo or one of three selected GHRH agonists: JI-38, MR-356, or MR-409. Researchers measured MI size, the number of cardiac c-kit+ stem cells, cellular mitotic divisions, and vascular density. Plasma levels of inflammatory cytokines were measured one week post-MI in the MR-409 group.

Results

GHRH agonists significantly improved the survival of nutritionally deprived H9c2 cells by decreasing calcium influx. In the animal model, treatment with GHRH agonists JI-38, MR-356, and MR-409 all successfully reduced the size of the myocardial infarction compared to placebo. This reduction was associated with an increased number of cardiac c-kit+ cells, more cellular mitotic divisions, and higher vascular density in the infarct border zone, indicating enhanced repair processes.

One week after the MI, the MR-409 group showed significantly reduced plasma levels of key inflammatory cytokines IL-2, IL-6, IL-10, and TNF-α compared to the placebo group. Gene expression analysis for the MR-409 group revealed the treatment worked by inhibiting pro-apoptotic molecules and pro-fibrotic systems while simultaneously elevating bone morphogenetic proteins (BMPs), which are crucial for tissue regeneration and repair.

Key Findings

  • GHRH agonists JI-38, MR-356, and MR-409 all reduced myocardial infarction (MI) size in rats.
  • MR-409 significantly reduced plasma levels of IL-2, IL-6, IL-10, and TNF-α one week post-MI.
  • Treatment increased the number of cardiac c-kit+ cells, cellular mitotic divisions, and vascular density.
  • In vitro, GHRH agonists improved H9c2 cell survival by decreasing calcium influx.
  • MR-409 inhibited pro-apoptotic and pro-fibrotic gene expression while elevating bone morphogenetic proteins.

Why It Matters

This research suggests that GHRH agonists like sermorelin and its more potent analogs could be a powerful therapeutic tool for improving recovery after a heart attack. For biohackers or clinicians exploring regenerative peptides, this highlights a specific mechanism: GHRH agonists may protect the heart by reducing post-MI inflammation and fibrosis, not just through GH stimulation. This creates a new rationale for using these compounds in cardiac recovery protocols. While these are promising animal results, human trials are needed to establish safe and effective dosing protocols for post-MI patients. The findings suggest that early administration of a GHRH agonist post-MI could significantly improve long-term cardiac function and remodeling by creating a more favorable healing environment in the heart.
