Mazdutide Trial to Evaluate Efficacy in Reducing Fatty Pancreas in Overweight/Obese Patients
Background
Fatty pancreas, or pancreatic steatosis, is increasingly recognized as a significant component of metabolic dysfunction-associated steatotic liver disease (MASLD) and type 2 diabetes mellitus (T2DM). It contributes to pancreatic inflammation, impaired insulin secretion, and increased risk of pancreatitis. Current interventions primarily focus on weight loss through lifestyle modifications, which often yield inconsistent results for pancreatic fat reduction. There's a critical need for pharmacological therapies that specifically target pancreatic fat accumulation and its metabolic consequences. Mazdutide, a dual GLP-1 and glucagon receptor agonist, has demonstrated substantial weight loss and improvements in hepatic steatosis, suggesting a potential role in ameliorating fatty pancreas. This trial aims to explore this therapeutic gap.
Study Design
This is a randomized, double-blind, placebo-controlled, parallel-group, single-center clinical trial. The study plans to enroll 194 participants with overweight or obesity, who will be randomly assigned in a 1:1 ratio to either the mazdutide plus standardized lifestyle intervention group or the placebo plus standardized lifestyle intervention group. The treatment duration is 48 weeks. The primary efficacy endpoint is the change from baseline in pancreatic proton density fat fraction (PDFF) measured by MRI at week 48. Secondary endpoints include changes in body weight, BMI, waist circumference, glycemic and lipid metabolic parameters, visceral fat, and hepatic fat content.
Results
This clinical trial is currently in the planning phase and aims to provide preliminary evidence-based data on mazdutide's efficacy. No results are available yet, as the study is designed to evaluate the impact of mazdutide on fatty pancreas. The primary objective is to quantify the change from baseline in pancreatic PDFF at week 48 using MRI in patients receiving mazdutide compared to placebo.
