Tirzepatide pilot trial aims to slow biological aging and improve physical function in older adults with obesity

Tirzepatide to Slow Biological Aging

Background

As the global population ages, the burden of age-related diseases and declining physical function becomes increasingly significant. Biological aging, distinct from chronological age, reflects the accumulation of cellular and molecular damage, contributing to conditions like sarcopenia, frailty, and increased susceptibility to chronic diseases. Current interventions to directly target biological aging are limited. Tirzepatide, a dual GLP-1 and GIP receptor agonist, is highly effective for weight loss and improving cardiometabolic risk factors, which are often intertwined with accelerated aging. This study explores whether these metabolic benefits extend to directly modulating aging pathways and improving functional outcomes in older adults.

Study Design

This pilot clinical study will randomize 90 older adults (aged 55-70 years) with an indication for tirzepatide weight-loss therapy. Participants will receive either tirzepatide 2.5 mg subcutaneously (SC) weekly or no drug for 24 weeks, followed by a 12-week off-drug period. The primary focus is not weight loss, but rather to document the effects of tirzepatide on markers of aging, physical function, and overall health. Specific assessments will include body composition, bone health, and physical performance metrics, utilizing advanced biological aging clock assays and functional tests.

Results

This is a description of a recruiting pilot clinical study (NCT07218445); therefore, no results are available yet. The study aims to investigate the effects of tirzepatide on biological aging markers, physical function, and overall health in older adults with obesity. The primary endpoints will be changes in these aging and functional parameters over the 24-week intervention period.

Why It Matters

If positive, this pilot study could represent a significant step towards repurposing tirzepatide as an anti-aging therapeutic, extending its benefits beyond metabolic health. A positive outcome would suggest that existing, highly effective metabolic drugs could also address fundamental aging processes, potentially delaying the onset of age-related decline and improving quality of life for older adults. This could lead to new protocols where tirzepatide is considered not just for weight management or diabetes, but also as a proactive intervention for healthy longevity. The findings could pave the way for larger trials investigating specific dosing and duration for anti-aging effects, potentially impacting how clinicians approach age-related health challenges.
