Background

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia, significantly increasing risks of stroke, heart failure, and mortality. Obesity is a well-established modifiable risk factor for AF development and progression, contributing to atrial remodeling, inflammation, and fibrosis. Weight loss interventions have shown promise in reducing AF burden, but pharmacological options specifically targeting AF in obese patients are limited. Semaglutide, a glucagon-like peptide-1 receptor (GLP-1R) agonist, is approved for weight management and type 2 diabetes, demonstrating substantial weight loss and cardiovascular benefits, making it a compelling candidate to investigate in this context.

Study Design

The SOCRATES-AF pilot study was designed as a prospective, parallel-group, double-blind, randomized controlled trial, aiming to assess the feasibility of investigating Semaglutide in atrial fibrillation patients. The intended population included individuals with paroxysmal or early persistent atrial fibrillation who were also overweight or obese. Participants were planned to receive either Semaglutide 2.4 mg or standard medical treatment, with the primary objective being to assess the feasibility of such a trial. A subsequent, larger study, the WAIT Study, is also planned to investigate the effects of Semaglutide 2.4 mg on AF severity and recurrence risk after standard AF treatments. However, the SOCRATES-AF pilot study was withdrawn with 0 actual enrollment, and the WAIT Study is currently not yet recruiting. Therefore, no data has been generated from these specific trials.
