Background

Type 2 diabetes (T2D) is characterized by impaired glucose metabolism, significantly impacting various organs, including the heart. While GLP-1 receptor (GLP-1R) agonists like liraglutide offer substantial glucose and weight-lowering benefits, emerging evidence also suggests direct cardioprotective effects independent of systemic glucose control. However, the precise mechanisms and the potential for synergistic action with insulin on myocardial fuel selection in T2D patients remain underexplored. Understanding this cooperation could optimize therapeutic strategies for cardiac health in diabetes.

Study Design

A randomized clinical trial will enroll 27 Type 2 diabetic subjects (HbA1c 7.5-9.5%) currently on diet/exercise or oral antidiabetics, all on a background of metformin 2000 mg per day. Subjects will be randomized to one of three treatment groups (n=9 each): insulin detemir, liraglutide, or liraglutide plus detemir. Treatment duration is 3 months. The primary endpoint, myocardial fuel selection, will be measured by PET scans under fasting conditions following the final treatment injection.
