Background

Polycystic ovary syndrome (PCOS) is a prevalent and complex endocrine disorder strongly associated with obesity, increased visceral adiposity, and significant insulin resistance. Current therapeutic approaches, including lifestyle modifications and off-label use of metformin, often yield only modest and unsustained weight loss, proving insufficient to reverse the underlying obesity-driven pathophysiology. Growth hormone (GH), primarily through its stimulation of free 'bioavailable' insulin-like growth factor (IGF-I), is hypothesized to enhance insulin sensitivity and improve the overall metabolic profile in women afflicted with PCOS. This study specifically aims to unravel the precise mechanisms by which low-dose GH treatment influences the body's response to insulin, as well as its effects on steroid hormone metabolism, including cortisol clearance and testosterone levels, in obese PCOS patients.

Study Design

This is a randomized, quadruple-blinded, crossover clinical trial designed to investigate the effects of low-dose GH in obese PCOS women. Eligible participants, not recently on GH treatment, will be randomized to receive either daily low-dose GH or placebo injections for an initial 12-week period. Following a 4-week washout phase, subjects will crossover to the alternate treatment arm for another 12 weeks. Comprehensive assessments will be conducted before, during, and after each treatment phase. These include scans to quantify whole-body, abdominal, muscle, and liver fat, along with blood tests to measure cortisol levels and insulin sensitivity. Fat biopsies will also be analyzed for IGF-I density. Insulin sensitivity will be a key endpoint, measured at all four visits to the Oregon Clinical and Translational Research Institute (OCTRI).
