AMPLI-PPHI Project Catalyzes Adoption of Effective Postpartum Hemorrhage Drugs in Six Countries
Background
Postpartum hemorrhage (PPH) remains the leading cause of maternal mortality worldwide, disproportionately affecting women in low- and middle-income countries. Despite updated WHO recommendations and the availability of effective interventions like heat-stable carbetocin, tranexamic acid, and misoprostol, their adoption and scale-up are severely limited by persistent supply and demand-side barriers. This critical gap prevents widespread access to life-saving PPH prevention and treatment, highlighting an urgent need for implementation-focused initiatives to bridge the divide between evidence and practice.
Study Design
The Accelerating Measurable Progress and Leveraging Investments for Postpartum Hemorrhage Impact (AMPLI-PPHI) project, led by Jhpiego in partnership with PATH and FIGO, was initiated to address these implementation challenges. Operating in six target countries—Democratic Republic of the Congo, Guinea, India, Kenya, Nigeria, and Zambia—the project focuses on increasing women's access to PPH prevention and management. It introduces and integrates proven approaches and PPH commodities (e.g., heat-stable carbetocin, tranexamic acid, misoprostol) into routine maternal health systems, generates evidence on implementation, and supports national scale-up efforts. Globally, AMPLI-PPHI works to reduce supply-side barriers to create a market for affordable and accessible PPH commodities and fosters a regional exchange network for shared learning.
Results
This overview details the AMPLI-PPHI project's comprehensive strategy to combat postpartum hemorrhage (PPH). The project, led by Jhpiego with PATH and FIGO, operates across six target countries: Democratic Republic of the Congo, Guinea, India, Kenya, Nigeria, and Zambia. It focuses on catalyzing the adoption of effective PPH drugs, including heat-stable carbetocin, tranexamic acid, and misoprostol, as part of quality intrapartum care. AMPLI-PPHI actively introduces and integrates these proven approaches and commodities into routine maternal health systems, aiming to increase women's access to PPH prevention and management. A key global focus is reducing supply-side barriers to foster a market for affordable and accessible PPH commodities. The project also establishes a regional exchange network to facilitate shared learning and best practices among participating nations. > The overarching objective is to transform PPH care and significantly reduce maternal deaths, ensuring universal access to life-saving interventions.
Why It Matters
This initiative is crucial for bridging the gap between proven PPH interventions and their real-world application, especially in resource-limited settings. It directly addresses critical supply and demand-side barriers that currently prevent widespread access to life-saving drugs like heat-stable carbetocin and tranexamic acid. By integrating these into routine maternal health systems and fostering regional learning, AMPLI-PPHI aims to accelerate the reduction of maternal mortality from PPH globally. This work directly impacts clinicians and health systems by strengthening infrastructure for delivering essential care, moving closer to a future where effective PPH management is universally accessible for every woman.
