Tirzepatide Outperforms Other GLP-1s in Preventing Cardiovascular-Kidney-Metabolic Disease Progression

Background

Cardiovascular-kidney-metabolic (CKM) syndrome is a complex, progressive condition encompassing type 2 diabetes, obesity, cardiovascular disease, and chronic kidney disease, leading to significant morbidity and mortality. While GLP-1 receptor agonists (GLP-1 RAs) are established treatments for improving glycemic control and reducing cardiovascular risk, their comparative effectiveness, especially that of newer dual agonists like tirzepatide, on the holistic progression of CKM stages in real-world settings remains underexplored. This study addresses the critical knowledge gap regarding how tirzepatide compares to other GLP-1 RAs in preventing CKM stage progression in a diverse patient population.

Results

Patients treated with tirzepatide demonstrated a significantly lower risk of CKM stage progression compared to those on other GLP-1 RAs. The tirzepatide group experienced a 38% reduction in the primary outcome of CKM stage progression (p<0.001). The most impactful finding was a 43% lower risk of advancing to a higher CKM stage in patients receiving tirzepatide compared to the pooled GLP-1 RA group over the 2-year follow-up period. Specifically, tirzepatide was associated with a 28% reduction in major adverse cardiovascular events (MACE) (p<0.01) and a 35% slower rate of eGFR decline, indicating superior kidney protection (p<0.005). Furthermore, patients on tirzepatide achieved an average 1.8% greater reduction in HbA1c and 6.2 kg more weight loss than the comparator group, highlighting its potent metabolic benefits.
