GLP-1 Agonists Linked to Increased Erectile Dysfunction Risk in Type 2 Diabetes

GLP-1 receptor agonist and risk of erectile dysfunction in men with type 2 diabetes: a target trial emulation

Background

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are widely used to manage type 2 diabetes (T2D) due to their well-established metabolic and cardiovascular benefits. However, the impact of these agents on men's sexual health, specifically the risk of erectile dysfunction (ED), has been poorly understood. Previous research provided very limited and inconsistent evidence regarding the association between GLP-1RAs and sexual health outcomes in men with T2D, highlighting a critical knowledge gap.

Results

The study found that initiating GLP-1RAs was associated with a modestly increased risk of erectile dysfunction (ED) when compared to initiating DPP4 inhibitors. This finding was consistent across multiple sensitivity analyses, reinforcing its reliability and suggesting the association was not due to unmeasured confounders. The association between GLP-1RA initiation and increased ED risk was externally validated in the TriNetX database, confirming the robustness of the primary findings across different data sources. This suggests a potential adverse effect on sexual health outcomes in men with type 2 diabetes receiving GLP-1RAs, despite their known metabolic benefits.

Why It Matters

This study highlights a crucial, previously under-recognized aspect of GLP-1RA therapy: its potential impact on male sexual health. The finding that GLP-1RAs may increase the risk of erectile dysfunction suggests that clinicians should consider discussing sexual health outcomes with men with type 2 diabetes when prescribing these medications. These insights could inform clinical guidelines, prompting a more holistic assessment of patient well-being beyond metabolic control. Further mechanistic and clinical research, including prospective human trials, is essential to understand the underlying biological pathways and confirm these observations.
