Liraglutide + Metformin Combination Investigated for Liver Glucose Reduction in African-American Youth with Type 2 Diabetes

Type 2 Diabetes (T2D) in youth, particularly African-Americans, presents unique challenges, with standard treatments like metformin not universally effective. Excess hepatic glucose production (HGP) is a key contributor to hyperglycemia in T2D. While Liraglutide, a GLP-1 receptor agonist, is approved for adult T2D, its efficacy and mechanism in reducing HGP in this specific youth population, especially when combined with metformin, remain underexplored. Understanding these effects could lead to more targeted therapies.

This randomized clinical trial enrolled African-American youth aged 12-25 with Type 2 Diabetes. Participants underwent an initial screening, followed by a 1-week washout of prior diabetes medications. They were then randomized to receive either liraglutide + metformin daily or metformin alone daily for 3 months. The primary objective was to assess the reduction in excess sugar produced by the liver, likely measured via isotope tracer methods during intensive overnight clinic visits, which included IV catheterization, blood draws, and breath tests. The study involved multiple clinic visits, including a repeat of the intensive overnight protocol at 3 months.
