Trial to compare semaglutide and tirzepatide with or without exercise on body composition in obesity
Background
Obesity is a global health problem leading to serious issues like cardiovascular disease, insulin resistance, and inflammation. While novel anti-obesity drugs like semaglutide (a GLP-1 receptor agonist) and tirzepatide (a dual GLP-1/GIP agonist) induce significant weight loss, concerns exist about potential muscle mass reduction. This muscle loss can impair physical function and metabolic health, potentially leading to sarcopenic obesity. This trial addresses the critical gap of whether structured exercise can mitigate muscle loss and enhance overall health benefits when combined with these potent medications.
Study Design
This NOT_YET_RECRUITING clinical trial (NCT06885736) plans to compare the effects of anti-obesity medications alone versus combined with exercise in individuals with obesity. Participants will receive either pharmacological treatment with semaglutide or tirzepatide, or a combined regimen including these medications plus structured resistance exercise. The primary objective is to assess changes in body composition, while secondary endpoints include cardiometabolic markers, muscle strength, functional capacity, and respiratory function. The study is scheduled to run from March 2026 to December 2029.
Why It Matters
Preserving lean muscle mass during significant weight loss with GLP-1/GIP agonists is a critical health goal. This trial aims to provide evidence on whether adding structured exercise to semaglutide or tirzepatide regimens can mitigate muscle loss and improve overall metabolic and functional health in individuals with obesity. If successful, the findings could lead to updated clinical guidelines recommending concurrent exercise with these medications, optimizing patient outcomes beyond just weight reduction. This could inform future protocols for combining pharmacological and lifestyle interventions, ensuring more holistic health benefits and potentially preventing sarcopenic obesity.
