Melanotan II, often referred to as 'Barbie drugs', is an alpha-melanocyte-stimulating hormone (α-MSH) analogue that can be readily purchased online and in gyms for sunless tanning. This peptide works by stimulating the production of eumelanin, a pigment responsible for darker skin tones. While previous research has indicated that Melanotan II may contribute to the development of new pigmented and dysplastic naevi (abnormal moles that can be precursors to melanoma), the acute systemic side effects following self-administration have not been extensively documented in medical literature.
Results
Upon presentation, the patient exhibited severe sympathomimetic symptoms (a set of physiological effects mimicking an overactive sympathetic nervous system, such as increased heart rate, elevated blood pressure, and anxiety). He received immediate medical attention, which included treatment with lorazepam (a benzodiazepine used to reduce anxiety and calm the nervous system), supplemental potassium, and intravenous fluid resuscitation to stabilize his condition. The most critical finding was the rapid onset of severe sympathomimetic symptoms within 2 hours of self-administration, necessitating emergency medical intervention and highlighting significant acute systemic toxicity. This acute presentation contrasts sharply with the previously emphasized long-term dermatological risks, revealing a more immediate danger.

This case report critically underscores the immediate and potentially life-threatening systemic risks associated with the unregulated self-administration of Melanotan II. Despite its widespread availability through online channels and gyms, users are often unaware of these severe acute dangers, focusing instead on its cosmetic tanning effects. Increased public awareness and robust educational campaigns for both consumers and healthcare professionals are urgently needed to prevent similar adverse events and ensure patient safety. This highlights a pressing need for stricter regulatory oversight and further research into the pharmacokinetics and acute toxicology of Melanotan II.
